Please keep in mind that you must tour the school before
your child can start school. Submission of the medical forms is not
required until Orientation Day.

The Certificate of Immunization is printable in Word format:
Certificate of Immunization
You must submit proof that your child had a physical examination within
one year of the start of school, and that your child's immunizations are up to date.
Your physician's immunization
and yearly physical forms are acceptable.
